Upon making an appointment, you will be requested to complete comprehensive intake forms that will help you to summarize your health history and current symptom picture. You will also be requested to sign our payment policy and an informed consent, explaining our physicians’ education and licensure as well as our clinic’s payment policies.

What to expect during your first visit or phone consultation

The initial visit is a time for your physician to review pertinent medical history and discuss current problems and symptoms. It is a time for gathering of information, as well as a time for sharing concerns and getting to know each other.

Finally, each patient will be presented with a comprehensive regimen of the necessary herbal remedies, homeopathics, and other nutritional supplementation that would positively impact a change in the problematic health condition.
